Your first visit
Four simple steps
- 1Choose your office. Pemberton Square or Hwy 61 North. Both are open Monday to Friday, 8:00 am to 5:00 pm.
- 2Book online or call. Use the online scheduler for your office, or call Pemberton at (601) 636-5321 or 61 North at (601) 629-6066.
- 3Fill out your forms. Complete your patient forms before your visit to save time.
- 4Bring what you need. Your dental insurance card if you have one and a list of any medications you take.
